HBA introduces the Hawaii Dental Service Plan which is available to all members.
Attached are forms to either fill out online or print and fill out manually, with a description of the plan from HDS. You must first email the HBA Dental Plan Aministrator to obtain a HBA Group and Division member number at:
Under this HBA plan, each division (HBA member) will handle any ongoing maintenance to enroll new hire employees, make changes for existing employees and to terminate employees. Monthly Fee ranges from $38 for a single employee, to $121 for a family plan for each employee, with a $2000 annual maximum limit, and a $25 copay. Each eligible employee wanting to enroll in the dental plan must complete an enrollment/change form (see attached). The eligible enrollees are the employee, spouse and dependent children covered through age 18 or full-time students enrolled in an accredited college covered through age 24. If a spouse or dependents are added, the employee will need to include them on their (HBA member employee) enrollment/change form. These forms can be mailed or faxed directly to our membership services department at 808-529-9207.
